Abstract The present paper reviews the pathophysiology and therapeutic strategies in heart failure based on clinical investigations performed in our institute, and also recently reported large-scale double-blind clinical trials for the medical treatments of heart failure. Careful introduction of beta-blocker therapy along with RAAS inhibitors (ACE inhibitor, ARB, and/or aldosterone antagonist) should bring about marked improvement in mortality and morbidity in all subgroups of patients with heart failure.
