政府主導「ジャパン・インターナショナル・ホスピタルズ（JIH）」の推奨病院として、3度目の認定を受けました。

　一般社団法人Medical Excellence JAPAN（MEJ）※は、日本の医療機関に外国人患者の受入れを促進するため、政府と協調し、外国人患者の受入れに意欲と取組みのある病院を「ジャパン インターナショナル ホスピタルズ」として推奨しています。
　2017年4月、多岐にわたる審査を受け、当院はJIH推奨病院として初めて認定されました。
　2020年4月に2度目の認定を経て、2023年4月に3度目の認定を受けました。
　受入診療科は循環器内科、消化器内科、脳神経外科、麻酔科です。
　当院の外国人患者受入れ体制に関する情報はMEJのホームページにも掲載されています。
　新型コロナウイルスの終息後には、人・モノ・サービスのボーダレス化が一層加速するものと考えられます。引き続き世界中の患者さんに安心・安全で質の高い医療サービスを提供し、JIH推奨病院としての使命を果たしていく所存です。

Nippon Medical School Chiba Hokusoh Hospital has been recommended consecutively three times as "Japan International Hospitals (JIH)" led by Japanese Government .

　Japanese government recommends hospitals that have a willingness and the ability to accept patients from different countries as "Japan International Hospitals".
　As a result of undergoing various examinations by Medical Excellence Japan (MEJ*) that is official bureau of JIH, this hospital has been certified in a row as a member of JIH in April 2017, 2020, and 2023.
　Enrolled departments to receive medical travelers are cardiology, gastroenterology, neurosurgery, pain clinic etc..

　 Now that COVID-19 is stamped out, the borderlessness of people, goods and services are expected to be accelerated more and more. We make still more efforts to provide secure, safe and high-quality medical services for medical travelers, to fulfill the mission as a member of JIH.

当院を受診される外国人の方へのご案内

1.医療通訳有料サービスについて

英語、中国語、台湾語、韓国語、他3か国語、計7か国語の通訳有料サービスを行っています。医療通訳有料サービスを希望される方は、事前にご連絡をお願いいたします。

2.渡航受診者について

事前予約制ですので、事前にご連絡をお願いいたします。
原則、和訳された医療情報（診療録、検査結果、画像データ等）をご準備ください。
なお、当院で実施する医療情報の簡易翻訳料は16,500円（消費税込）です。

3.（オンライン）セカンドオピニオンについて

専門領域疾患の診断内容や治療法に関して専門の立場から意見・判断を提供いたします。
事前予約制ですので、事前にご連絡をお願いいたします。ご本人の相談を原則といたします。
委任状（同意書）をお持ちであれば、ご本人以外の相談が可能です。ただし、委任者・受任者確認のため、各々顔写真の入ったID（パスポートや在留カード等）が必要です。
原則、和訳された医療情報（診療録、検査結果、画像等）をご準備ください。　

料金は以下の通りです。
30分まで33,000円（消費税込）、30分を超える毎に33,000円を加算
※ オンラインの場合、30分まで44,000円（消費税込）、
　 30分を超える毎に44,000円を加算
※ 診察や家族相談に移行となる場合があります。

4.診療について

健康保険証をご持参ください。
健康保険証をお持ちでない方は顔写真の入ったID（パスポートや在留カード等）をご持参ください。
原則、他医療機関からの紹介状（診療情報提供書）をご持参ください。お持ちでない場合、保険外併用療養費7,700円（消費税込）のお支払いをいただきます。ただし、緊急その他やむを得ない事情により、他の医療機関等からの紹介によらず来院した場合等にあっては、この限りではありません（厚生労働省告示）。

5.その他

当院はジャパン・インターナショナル・ホスピタルズ（JIH）に選定されており、受診者の国籍や対応言語等に関する統計を報告しなければならないため、それらをお尋ねする場合があります。

Guide for patients from different countries

1.Paid Language Support Service

English, Chinese, Taiwanese, Korean, and other 3 languages are available. Please contact free to us if you need the service in advance.

2.Medical Traveler

Advance reservation is required.
Please prepare medical materials translated into Japanese that are medical record, laboratory data, medical imaging and so on, if possible.
Simplified translation by us costs you 16,500 yen.

3.Second Opinion

You can seek medical and professional advice on the content and course of treatment given, in the specialized field.
Advance reservation is required.
Essentially, patient attendance is required. The recipient of a mandate can attend with a letter of attorney, provided that 2 and over IDs for each must be confirmed.
Please prepare medical materials translated into Japanese that are medical record, laboratory data, medical imaging and so on, if possible. Simplified translation by us costs you.

The fee of Second Opinion	33,000 yen in 30 minute units
The fee of On-Line Second Opinion	44,000 yen in 30 minute units
The fee of simplified translation	16,500 yen
The fee of translated summary documentation	5,500 yen+11,000 yen per page

Second opinion may be sifted to providing ordinal medical treatment or consultation of it.

4. Preparation

Please bring ID with your photo such as a passport and Residence Card if you donʼt have Japanese insurance policy.
Please submit a referral letter from other hospitals or clinics in principle.
Unless you bring it, the additional fee 7,700 yen is charged basically, except for an emergency case.

5.Others

This hospital is authorized as “Japan International Hospitals(JIH)”  and is obliged to submit the data on the patients from different countries. Therefore, please understand in advance that we ask them the nationality, mother tongue, etc.

外国籍患者就诊须知

1.关于医疗口译及收费

本院提供英文，中文（北京话，台湾话），韩文，及其他3个语言，共7个语言的医疗口译服务。该服务为收费项目。就诊时，需要医疗口译服务的患者，请事先联系我们。

1. 2.关于赴日就医患者

2. 本院可以应对赴日就医的患者，但需要事先预约。
   原则上需要提供给本院，日语的医疗信息（患者病历，各项检查结果，影像资料等）。
   希望在本院就诊的赴日就医患者，请准备好以上医疗信息，提前联系我们。

1. 3.关于（远程）第二诊疗意见

本院以专家立场，针对专业领域各种疾患的诊断内容及疗法提供第二诊疗意见。并且可以远程（联网）进行。该服务需要事先预约，请您提前联系我们。
原则上，仅限患者本人或家属咨询。如果有委托书，可以代理咨询。本人不在日本，只有代理人来院时，为确认请携带患者本人，代理人双方的身份证明证件（护照或在留卡等带照片的证件）。
　此外，需提前提供日语的医疗信息（患者病历，各项检查结果，影像资料等）。
费用如下；
・咨询第二诊疗意见：    33,000日元/30分钟（含税），延长加算
・远程咨询第二诊疗意见：44,000日元/30分钟（含税），延长加算

1. 4.关于就诊

　来本院就医时，请携带好健康保险证。
没有加入日本国内健康保险的患者，请携带身份证明证件（护照或在留卡等带照片的证件）。
在本院就诊，原则上需要其他医疗机构的介绍信。如果您没有介绍信，须支付保险外并用疗养费7,700日元（含税）。根据日本后生劳动省公告，急救状况后其他特殊情况可以除外。

5.其他

本院作为Japan International Hospitals (JIH)，会收集来本院就诊的外国籍患者的国籍，语言等信息。请事先予以理解。
